Good Samaritan Faith Formation

Faith Formation for Children and Youth

At Good Samaritan, we join you in the responsibility to raise your children in the practice of the faith. Knowing Jesus helps our children to grow in relationship with him, and learning about the Catholic Faith helps them discover how to live lives of faith. Our monthly faith formation is designed to help your children grow in faith with your oversight and guidance. We have curriculum that your children will go over with you and together you can share your love of the Lord and His Church.

Is this a Sacramental Year for your Child?

First Reconciliation and First Eucharist:

All children and youth preparing for sacraments need a foundation in faith formation when coming to the sacraments. Parents will need to discern if their child is ready to begin preparation by prayerfully reviewing the questions below.  After completing preparation, parents will join catechists and staff to determine readiness of your child to receive sacraments.  Preparation for the sacraments is done in addition to ongoing faith formation appropriate to the grade level of the candidate for the sacrament.   

1. Parental Discernment: To help you discern if you child is ready to begin preparation for First Reconciliation you should carefully consider the following questions:

  • Has your child been baptized and reached the age of reason; 7 years old?
  • Does your child know the difference between right and wrong?
  • Can your child distinguish between mistakes or accidents and deliberate wrongdoing?
  • Does your child express sorrow for wrongdoing?
  • Does your child forgive others?
  • Does your child pray at home and at Mass?
  • Does your child relate to Jesus as one who forgives?
  • Are you committed to the sacramental preparation process and having your child attend every preparation session?

Note: if you are not registered with either church please fill out the parish member form found on the Good Samaritan Page, the newcomer form found on the St. Gabriel home page or you can pick one up at the church.

Emergency Medical Treatment Release

Emergency Medical Treatment: In the event of an emergency, I hereby give permission to transport my child to a hospital for emergency medical or surgical treatment. I wish to be advised prior to any further treatment by the hospital or doctor. In the event of an emergency, if you are unable to reach me at the above numbers contact:

Other Medical Treatment: In the event it comes to the attention of the parish, its officers, directors and agents, the Diocese of Richmond, chaperones or representatives associated with the activity, that my child becomes ill with symptoms such as a headache, vomiting, sore throat, fever, diarrhea, I will be called and will pick up my child when notified.

Medications: My child is taking medication at present. My child will bring all such medications necessary and such medications will be in the original container. Names of medications and concise directions for seeing that the child takes such medications, including dosage and frequency of dosage are as follows:

Specific Medical Information: The parish will take reasonable care to see that the following information is held in confidence.

3. WE MUST HAVE A COPY OF YOUR CHILD’S BAPTISMAL CERTIFICATE ON FILE BEFORE THEY CAN RECEIVE ANY SACRAMENTS.

Sacramental Preparation for Confirmation is held on the third Sunday of each month through April.

1. Parent and Confirmandi Discernment: To help discern if your youth is ready to begin preparing for Confirmation, have them consider the following:

  • Have you received the Sacraments of Baptism, First Reconciliation and First Eucharist?
  • Are you at least 15 years old and in the 10th grade of High School?
  • Are you interested in receiving the sacrament?
  • Do you feel you have a basic knowledge of the Catholic faith?
  • Do you have the desire for ongoing conversion and deepening relationship with Jesus Christ?
  • Do you participate regularly in the sacramental life of the church including regular mass attendance and reception of the Holy Eucharist?
  • Are you prepared to commit to the requirements (attend all faith formation classes, attend all Confirmation Preparation class, attend a  Diocesan Confirmation Retreat) to receive the sacrament of Confirmation?
