Instructions

1. Review the Ministry Descriptions in the Parish Ministry Opportunities section of the Parish Handbook.

Click on Ministry Descriptions to view the Parish Handbook. In the Table of Contents, click on Parish Ministry Opporunties. The list of opportunities contain Ministry Descriptions. The ministry opportunities are listed in the same order in the Parish Handbook and on the Covenant Form. (You can view the Parish Handbook online, save it to your computer and/or print it).

2. Print, fill out and return the Time and Talent Covenant Promise Form.

Click on Covenant Promise Form below to view and print the form. This is the form you use to check off the ministry or ministries you choose... the ministry opportunities are listed on the front and back. (You cannot fill out the form online; you must print it and then fill it out).

3. Check off the ministry or ministries you choose on the Covenant Promise Form.

• If you are continuing in a ministry, make sure to check it off. We will not presume you are staying with a ministry unless you check it off.

• Print and use a separate form for each individual in your household who wants to choose a ministry or ministries.

4. Sign and provide the information requested at the top of the Covenant Promise Form and return the form.

Put the form in the collection at Mass, into the office mail slot in the Commons or bring it/ mail it to the Parish Office (see the address and phone number on the form).
